The Geriatric Assessment Clinic / Seniors’ Health Clinic is an outpatient clinic serving older adults who live in the community. If you live at home, a retirement home or a long-term care facility and can come to the clinic, you will be seen by a geriatrician or a nurse practitioner who will complete your assessment, make recommendations and be in touch with your family physician with the results of your visit.
OUR COMMUNITY OF PRACTICE CONNECTS YOU WITH OTHERS WHO SHARE YOUR PASSION FOR FALL PREVENTION.
We inform, share ideas and support each other to improve the implementation of evidence-informed fall prevention practices.
We are the voice for fall prevention locally, provincially and nationally.
