Appeal: SGFP COVID Vaccination in Long Term Care Survey all

In an effort to provide local Public Health Units (PHUs) with a list of names to help with the phase 1 vaccination effort in Long Term Care (LTC) and Retirement Homes (RH), we are collecting information in case you are called upon to support. During this phase, your local Public Health Unit is leading the vaccination distribution strategy.  

Please fill out this brief survey to help us understand your willingness and capacity to support Phase 1 efforts, should it be needed, as well as if you are already connected to and supporting residents living in a RH/LTC. You only need to fill this out once if you are a member of two or more associations.
